What you will need for your appointment:
- Patient intake form
- Proof of parenthood or legal guardianship
- Contact info for previous or referring physicians
- Referring physicians' medical records, results from laboratory or imaging studies (For example, X-rays, ultrasound, MRI, etc.)
- Insurance information, insurance card and pre-authorization (as needed)
- A list of all current medications and dosages (of current/ previous medications) or the bottles of medications

Your Coverage & Responsibilities
As a patient, you are responsible for providing us with accurate and complete billing information. We work with many insurance providers and plans and will be much more efficient and beneficial to you when we have complete and accurate information. Our billing department seeks to file reimbursement claims as quickly as possible. In addition, you are responsible for any deductibles and copayments at the time of your visit. Non-payment fees not covered by the insurance provider will also be your responsibility. This also applies to HMOs and PPOs. It is up to you to understand how your policy works and what rules and regulations you are subject to.

- Limits on charges
- Specific locations or physicians to use
Because insurance is a constantly changing field, we recommend that you stay up to date with your specific coverage. Be sure to provide your current insurance plan and policy number when you call in for an appointment.
